Rebound hyperglycemia

Question for fasting experts. I completed 77 hours of fasting. My morning glucose levels were below 100 every day I was fasting, lowest 62, highest 92. Ketones ranged from 2.5 to 4.9.

Morning after breaking my fast my glucose was 160. This morning it was 130. Typically my my morning glucose ranges between 90 and 110, sometimes a little higher, which I attribute to dawn effect, as my last A1C was 5.3.

Why would my post extended fast am blood sugars be so high? Has anyone experienced this? Am I doing more harm than good if I’m going to have rebound hyperglycemia after fasting?

Guess the first thing we need to know is what did you break the fast with?
My BG usually overreacts the first time I eat after an extended fast then settles back into its usual routine. If you think about your pancreas has effectively been asleep for three days then suddenly gets asked to deal with food so it goes “yikes” and overdoes it a bit… then gets used to being awake again and carries on as usual…

You would think if my pancreas was in overdrive the glucose would be low. I was thinking the opposite, that not enough insulin was being pumped out. But maybe is the liver pumping out glucose.

I had ribs, chicken, brussel sprouts, broccoli, and some dark chocolate. I probably made a mistake in eating too many carbs and need to keep it closer to zero carb for a couple days post fast. Last night I had a cheese and meat plate, pork rinds with smoked whitefish dip and jalapeño/artichoke dip, and NO chocolate. Glucose 130 this morning.

True… maybe my thinking is wrong and your pancreas is slow to respond… that does seem quite a large meal to break a fast with tho … I try to have something small at first to ease the body back into eating food… like an omelette. I have done 4 7 day fasts and found that by having a small meal my BG was better… I once broke a fast with a very large lunch and regretted it afterwards as it led to bloating and high BG.

Well, I ordered tons of food but the amount I ate was not a lot. I ate until I was full, and felt good after, no GI symptoms at all. But I should learn from this I guess and plan a simpler fast breaking meal of one or two ingredients, and maybe no veggies.

I should add that I am not diabetic, but I have PCOS and insulin resistance. My last A1C was 5.3, it was down to 4.9 at one point after a couple months Keto and was 5.6 pre-Keto. I’m on Metformin XR 750mg daily.

What was the grams protein per kg lean body mass per day consumed?

I don’t really know, I haven’t been tracking that any more. I could track again for a while, and track strictly my next fast. My previous goal was 60 grams a day, but I think 60-80gm would be ok. That would be 1gram/kg lean mass. Although I haven’t had a DEXA so I don’t know my exact LBM number.

If I had to estimate I would say that first meal was 60 grams of protein. The next day I probably had 80 or 90 total. Again, a guess.

Looks like protein was in good range.

Is it possible that you successfully reduced your insulinogenic response, so that you are secreting less insulin? This would lead to higher glucose, if you eat some carbs. If this is so, perhaps eating zero carbs, and monitoring your glucose levels, can help to provide more insight in this.

The reason I’m bringing this up is because I recently listened to Keto talk with jimmy Moore and Dr Nally. The doctor described this very phenomenon with some of his patients. His perspective was that this dynamic was a good thing, and not detrimental to health. Perhaps you may want to listen to that podcast?..I found it interesting that this can happen to some people.

I will listen to this. That is my suspicion, that I was secreting very little insulin and it took my pancreas a couple days to catch up. Next fast I will learn from this experience and choose my fast breaking foods very carefully.

And…when I am pregnant this is the very reason I will refuse the glucose tolerance test, because I very literally would not tolerate glucose while Ketogenic.
