Keto and the long term health risks of constipation

Hi there,

My husband and I have been on Keto since May 5 (seven weeks) and for the most part we have really loved the changes we are seeing in ourselves. He’s lost about 16 lbs and his belly is disappearing and I have lost 13 lbs. We started Keto after I was diagnosed with Fatty Liver Disease with NASH and was told by my GI and Hepatologist to lose weight. So far Keto has felt like the most sustainable way of eating to get the weight off and keep it off. It’s more than just vanity for me, I do not want the NASH to increase and for me to need a liver transplant. Also I was told I was on the edge of becoming pre-diabetic and I do not want to go down that path.

For the most part, we love how Keto makes us feel. I have less stomach distress in general, we are doing sort of lazy IF, which means we try to fast for about 16 hours, but I have a small glass of milk at bedtime and my husband drinks coffee with half and half in the morning before we break the fast at noon with solid food. We love that we can go longer periods without being hungry, that we aren’t slaves to cravings. Everything is good except for one thing: we are both constipated.

I have always been prone to constipation, but my husband was a guy that would go a few times a day, no problem. At first on Keto, we just seemed to need to go far less. But in the past few weeks it’s become a situation where we have less urge to go and when we do go, it’s not very smooth sailing and our BM’s don’t feel complete and leave us in some discomfort. Not pain, but just a physical sensation that not everything came out. (TMI, amirite?)

So, to sum it up:

-Less bowel movements overall
-Less volume
-Less comfortable BM, stool is small and hard, like rabbit poo (from gigantic rabbits) instead of the one piece banana shape.
-Less complete BM, feels like we can’t expel everything and stuff is left behind.

My question isn’t so much what to do about it, although we are open to suggestions. But rather, I want to hear how this is rationalized when discussing the long term safety of Keto. A diet that constantly has you constipated and requires possible intervention (laxatives etc) to have normal BM’s doesn’t seem like the perfect diet. This is the one thing that is making us question it and wondering if we should increase our carbs to include more fiber and maybe just having a lower carb diet instead.

I am currently drinking about two liters of water per day, I know I need to increase this. I sometimes try Miralax and sometimes it helps a little, but mostly not. We eat a lot of veggies. I do like dairy and feel like I need to eat less of that to help with both my weight loss and the constipation. My GI will have a fit when I tell him I am not going everyday, he actually wanted me to try Linzess before I even started Keto, when I first saw him for digestive problems and constipation.

What are your thoughts about the long term safety of Keto if one of the downsides is chronic constipation?

Constipation on Keto is largely due to magnesium shortage. Try supplementing with magnesium caltrate and see if this helps. It cleared it right up for me.

Also, try switching from half and half to HWC and something like an unsweetened vanilla almond milk instead of cows milk for your nightly glass. Neither of these are for constipation, but both swaps are much more Keto friendly.

You didn’t mention how much water you drink. Upping your water intake may help.

Anecdotally, and 1+ year into LCHF, this hasn’t been a problem for me. Well, my BMs are far more regular now than when I was eating all that carbage. Dr. Westman prescribes Milk of Magnesia if you find yourself with difficult BMs. I think that’s what @Anniegirl9 recommended?

Nope. I take it in supplement form. 2 tablets a day.

I am trying to drink water all day. We both need more. My doctor wants me to have about four liters a day, I am currently drinking about two.

I just bought some Magnesium Citrate by Soloray, 400 mg. They say to take 3 caps a day, so that would be 1200 mgs.

Does your doctor know you are doing Keto? Too much water can flush out your electrolytes, which are essential on Keto.

4 letters seems a bit high to me. Maybe someone else might know that better than me.

But if you are drinking more, you will need to supplement your electrolytes more. Salt, magnesium, potassium. Careful with the postassium.

Go with it and see how it goes.

When I take mine, I get a little bit nausous for about 5 minutes. You can break up your doses through the day if it upsets your stomach.

It takes a bit of mindfulness and discipline and prep but carry water with you on your daily routine if you can. And make sure you have extra.

I consume about 4 liters a day. Sometimes more. In the evening while watching tv I always have a glass of water next to me. Sip away.

My first couple of months Keto I had some constipated days, but not many. After almost a year they are now rare.

I’m glad I didn’t make the move for a supplement, although I get magnesium from a pill/drink bone broth/ACV, and just rode it out. Keto for me is not “a diet that constantly has you constipated”, I’m glad to say…

I hope you give it a chance and reevaluate after more time.

Both doctors know I am doing Keto. The liver doctor was enthusiastic about low carb and IF, my GI doctor didn’t specify anything other than he wanted me to lose weight. But I think he wants me pooping everyday. :slight_smile:

That’s awesome. Wish I could find that.

Then I would definately follow their suggestions on water.

This may mean I’m not getting enough myself.
Still, watch those electrolytes. Excess water can wash them out.

You sound a lot like me. A revelation for me is to eat LESS fibre and LESS veg. It works a treat and actually helps things move easier. Fibre can irritate your bowels. I was a sucker for the “eat more fibre” message my whole life but I realise less is actually more! I’d love to eat plates of veggies but I now limit myself to maybe a couple of mushrooms with brekkie, some broccoli and a small salad.

Try less veg for a while and see if it helps.

I had a kidney stone a few years ago. They ran the tests to try to figure out why I developed it in the first place, and basically their feeling was that I did not drink enough water. I try to keep up with their recommendations but it’s hard.

I’m lucky I live in Los Angeles and have access to really good doctors. My liver doctor is at Cedars Sinai.

The same for me, some constipation to start but it cleared up, I’ve been eating keto 18 months. My hypothesis is that your gut and the bacteria maybe have to adapt to the different food you’re sending through. It takes some time but I don’t have any issues any more. More water helped at the start.

If you drink too much water two things will happen

first you will dilute your stomach acid and end up with improper digestion.
second you will leak out your electrolytes.

both can cause constipation

I am against drinking water if your body does not ask for it.

Drink when you are thirsty.

Hi. I had the exact same experience too, and can totally relate to every single bulletpoint. I’m happy to say that I’m not having this problem anymore. What really helped me was having apple cider vinegar with water and lemon twice a day - I can’t believe the difference this made!
The constipation issue really really bothered me and I found others just didn’t grasp how unsettled and agitated it made me feel. Had it continued I’d have increased my carbs somewhat because I could not stand it, so I understand well your pondering on the diet’s efficacy. I’d your doctor says it’s ok it might be worth giving apple cider vinegar a go.

Hi there. I am over four months into the keto way of life now and I just stopped being frequently constipated. I was/am drinking plenty of fluids, drinking ketoade twice a day and I take magnesium supplements with a multivitamin at bedtime. I get plenty of green leafy vegetables, too. I can’t tell you exactly what changed other than I had only one ketoade per day (maybe unrelated) but the last few days have been much, much better. Maybe it will get better in time…make sure you get enough sodium and water. Good luck!
