Morning BG, Ketones and Inflammation

So I’ve been on Keto this round for about 2 months and I have a rather curious question. My typical fasting morning BG is usually around 105 ( sometimes a little higher ) and my ketones are around 2.5-3 mmol. When I do an extended fast ( 36 or 48 hours ) I’ve noticed that my fasting BG goes into the high 80’s and my ketones are usually 1mmol. I’m a really big fan of Doctor Boz on youtube and she talks about trying to get your ratio ( BG/Ketones ) into the 40-80 range for weight loss and <40 for autophagy. I’m a male in my early 40’s with no health issues other than being overweight and VERY insulin resistant ( pre-diabetic ). I have noticed some signs of inflammation… like rosacea ( beginning stages on the cheeks ) and some more specific aging male stuff that I won’t go into here. Also my brain fog / concentration really needs improvement. My blood pressure is really good and overall most things seem going good on keto.

I’m just curious why would my Ketones would go DOWN when I do extended fasts… or conversely why does my production of Ketones RISE when I eat more regularly yet are paired with HIGHer BG levels? I like measuring and tweaking and just looking for some thoughts on this.

Do you believe you are fully fat-adapted yet? That might be relevant in this connection. For one thing, if not fully-fat adapted, your body might be struggling a bit to keep everything supplied, and ketone use might be high enough while you are fasting that not much is left to circulate in your bloodstream.

I know people feel the BKI is important, and I respect Dr. Bosworth, but I’m not sure we really understand enough yet about what’s going on to be too certain about such things.

If you are feeling good while fasting, then keep doing it, but if you are hungry, or not feeling well, then eat. Plenty of time for fasting down the road, so you won’t be losing out.

P.S.—What are your ketone and glucose levels at other times of the day?
