I’d suggest also reading blogs and emails by Dr Jason Fung. The issue he highlights is insulin response.
Some people may have a “normal” glycaemic response to a given carb dose, but as they have reduced insulin sensitivity, it requires a large insulin response. I suspect that this is missed with the typical oral glucose tolerance tests.
