May be a stupid question, but from what I’ve read, most of the triggers that make you eat are because the body is telling you it’s TIME to eat, not that you’re hungry, so how do you tell when you are TRULY hungry?

Hunger pangs in the stomach. Due to ghrelin release. Other things, especially energy balance reactions are mainly blood sugar issues having to do with lipogenic hormonal resistance reduction. Once that state is normalized, you’ll only experience true hunger.
With Carb based patterns, people mostly eat due to dropping blood sugar rather than actual hunger.
Wait for a growl.
Sometime I feel hungry after a meal, but might be acid or digestion. . My head knows I’m full.

I think that answer is relative to were on the keto journey you are.
In the first month or so I still had the hormone, mental, time frame trained I am HANGRY response.
6-8 weeks it was more along the lines of mental I should be hungry because me stomach is growling but not always.(about 80% of the time I was not HANGRY vs just kind of hungry)
I fat adapted at 8 weeks or so and the response is now more like a low level pain if I were to describe it. It does not always follow a schedule, sometimes I get it in the morning, sometimes in the afternoon. It is worse when I eat earlier without being hungry (like yesterday I made a early breakfast which I don’t normally because of travel) or were I may have eaten a smaller amount than normal at the last meal when I am heavy on fat.

For me its knowing ‘how to eat’ to comfortably get to the next meal whether its OMAD, ADF or OM3D and etc.
I agree, I did a 9 hour trek through snow yesterday not knowing when I could eat again. So I ate when I was not hungry and not wanting to getting the 18 hour mark were I normally get hungry. Only flaw in the plan was my daugher ate half of my eggs the day before so I only had 1 egg and 2 pcs of bacon, so I under ate and low and behold I was hungry at my normal eating time 6 hours later. Thankfully I found and arbys and hung out for 2 hours while they plowed the roads.
5 months ago I would have had to stop twice in that time frame even when I was keto and eat full meals.
OMG, I wonder if this is what I was experiencing? Last night I had already gone to bed but I got this pain deep in my stomach, like below my sternum but several inches down (straight smack in the middle of my stomach). It ALMOST felt how hunger pains feel and I was waiting for a rumbles to come with it but it never happened. Just a pain. Anyway, just forced myself to go to sleep because I wasn’t getting up at midnight to eat.
So I wake up this am and still, there is that pain in my stomach. When I got to work I ate like a 1/4 cup of pecans and had some cream cheese on some cucumber sliced and drank some water. Now that feeling seems to be gone or only very slightly there.
I wonder if that was truly a hunger pain(s)? I was just used to the moodiness and grumbles that usually go along with it and had neither. It almost had me worried cause it really hurt. Hmmmm
I am very fat adapted, the hungry signals did not disappear for several months.
What you are describing sounds more medical issue I would get it checked out.
I’m not too sure it’s medical. I seem fine now that I ate but I certainly will be mindful of it. Thanks!
