Just watched that lecture. It was fascinating, but from some of his comments along the way, I’m not sure he entirely gets the point of a well-formulated ketogenic diet. Not only that, he treats his patients with liquid meal replacements, for some reason that he did not explain (would love to check the ingredients list on those). Do epileptics need exogenous ketones because they can’t make enough of their own?

Some of the epileptics have to be kept at ridiculously high ketone levels and they will do an extreme form of the ketogenic diet. The 4:1 diet is 4 grams of fat per 1 gram of protein and carbs combined, ie 90+% fat. And they do this with children who can be reluctant to adopt this way of eating. Thus the meal replacement shakes and other tricks.

I found it interesting that they have a lot of tests to evaluate what is going on metabolically and they have a fairly good idea of when keto will work and when it is harmful.
