Advice please - T2DM 3 weeks in w/ medication issue

Hi all,

I am requesting advice from those folks who have been keto for a while and are/were T2DM. Here’s the situation.

I have what Dr. Lustig calls the metabolic syndrome – hypothyroid, HTN, T2DM, hyperlipidemia, and obesity. I take 4 DM meds (Victoza, Lantus, metformin, & glipizide), 3 HTN drugs (Verapamil, Losartan, & Maxide), plus Synthroid. I want to get off the DM & HTN meds and realize it will take time. I have been decreasing my Lantus slowly, starting at 60u/day to 45u (last changed 2 days ago).

I OD’d on glip yesterday (forgot to take my am dose and took it at lunch (1pm), then took the 2nd dose last night around 6:30pm. 2 hours after dinner I was having a hypoglycemic rxn, and my BS was 70. I ate a snack (nuts), and an hour later it was 79, but I still had hypo symptoms. So I took a spoonful of agave syrup to prevent it from dropping more before going to bed. It was in the mid80s when I got up in the night to pee, and then it was still mid80s when I tested before breakfast. Decided not to take glip this morning, but did take the rest of my meds. Just tested again and my BS is mid-90s.

So, if my BS stays in the 80s & 90s today, do I not take my glip this evening? I don’t want to increase my carbs (now 20-30g/day), as the whole point of going keto is to reduce/get off my meds. I did call my GP & got the Dr on call, who thinks keto is a really bad idea for DM; my sister who is Type 1 also thinks keto is a bad idea. But I see plenty of folks here who are having great success reducing and getting off their meds.

Any advice re how to deal w/ my glip and lower BS is welcome!

Hi AnnM, With this many medications you definitely need to talk to your doc with what you’re doing and how to manage it. They know your bigger picture better than anyone on this forum does. Hopefully they’ll provide you with all the support you need.

They work remotely with patients and their local healthcare providers.

@brownfat, thanks so much! I will check them out. I’m doing OK w/ my Lantus as my former endocrinologist had discussed w/ me how to increase or decrease my Lantus, depending on my BS. But glipizide, I really don’t know. As it happens, the longer I am keto’ing, the lower my BS is going, so I will be decreasing my Lantus by 5 units tomorrow morning.
