Urine test

Hello I need some insight about my last couple Urine test.
I have been on keto for about 2 months now with a couple bad keto days 3 days before last Random Urine test. Both test were fasting 12 hours before test.

The first Urine test before I started keto : Microalbumin 3.4 REF .0-1.6
Urine Creatinine 56.9 REF 39-259
ACR: 59.8 REF .0-29.9
I might add that my A1C at that time was 7

These Test were performed at and by my Internal medicine doctor

The second Random test was recently 2 months on keto by primary care doctor
Microalbumin 3.8
Urine Creatine 20
ACR 189.1
A1C 6.3

Why did my second Urine test come back so bad on Keto? espectiall the Low Creatinine number?
That ACR is now terrible. I thought keto was supposed to help all of this stuff. Am I hurting my kidneys on Keto? A1C was better but Kindney function was much worse.
I will add that GFR remained roughly stable at 75 for both tests and Bun was 16 first test and 20 on second.

Please what affects might keto have on these test results.
Now I dont know what to do. Continue on Keto or not?

My advice would be to wait and see. The first six months on keto, everything is in flux, so tests done before then are generally not of concern.

I notice two things: first, your HbA1C is dropping, which is excellent for your kidneys. Also BUN is pretty much the same and GFR is stable. Those are the numbers I’d be most concerned about. Creatinine is going to look odd until you are fully fat-adapted. That generally takes two months, but it’s not an on/off condition, and you are likely to continue seeing improvements.

So as I mentioned, the best thing is to wait until the six-month mark before becoming too concerned.

Greetings @Kroman. Just trying to better understand your situation…

What are “random” urine tests? Typically that suggests job-related drug testing, but in both cases you were fasting for 12 hours beforehand … which isn’t necessary for drug-testing nor would that seem to make the test very “random”?

Your HbA1c at 7 suggests your physician is advising you re: diabetes. If so, your declining A1c indicates what you’re doing is off to a good start. Which makes me wonder: what do you mean by “a couple of bad keto days 3 days before”? Are you sticking to carb-restriction?

Random Urine test just means one sample is given that day at the doctor visit versus a 24 hour collection of Urine. Yes you are well aware ahead of time that a rountine MicroAlbumin Urine test will be done at your office visit as well as other labs and are told to fast for 12 hours before visit.

As far as Carb restriction I was doing really good for 2 months but gave in to temptation at the pushing by friends during a 3 day sporting weekend. I guess I was weak willed. But I am back on now.

Thanks for the clarification. Never heard that term random used in that context but now I know.

Whether or not you’re weak-willed (old habits die hard, especially involving food!), it doesn’t sound like your friends are as interested in your efforts to improve your health as true friends ought to be. Just my $0.02.
