Testing - everything is out of wack

Per Daisy (sorry Daisy, it would only let me tag two people), I’m sharing my post from a FB page. She thought it would be beneficial to get more eyes on this, in particular she mentioned @DaveKeto and @richard

Ok… soooooo… had my doctors appointment (if ya remember, I thought my blood sugar was fine because… well, I just assumed it would be ok again on Keto - it was the first time, but alas, when I tested it a couple weeks back, it was over 300. Still eating Keto, my endo appt is in 2 weeks - oh and I currently am not taking meds), and the results are in… fasting blood sugar was 311, A1C - 10.5, Sodium and Chloride were a little low.
Lipid panel:

Ok, so with that in mind, I’ll be going to the endo in a couple weeks. I’ll be back on meds, of course. Now, I know that my Triglycerides are related to my elevated blood sugar… but could it be that indicative of uncontrolled blood glucose? I don’t eat sugar/carbs(you know, except keto carbs)/alcohol, but do I now need to stop w/ butter or bacon drippings? Argh, this body of mine sucks.

Ok, so to explain the trend, in Dec. 2014 I was following the SAD and wasn’t taking many meds yet… I was just starting w/ my endo. In April 2015 I had just started keto like a month before. And then in Nov, I had been on Keto since April pretty consistently (but still with small doses of insulin, and meds).
and then shortly after Nov, I just fell off the wagon. Dad had died in Sept and food is my “torture” mechanism. I went out of control, ate all the sugar, lol. I’ve been on Keto since Jan, pretty consistently (yes, I’ve had some moments of weakness, but I’d say 95% Keto). I’ve lost 20 lbs since Jan.

I originally started Keto 3/2015 @ 225 lbs, I got down to 187ish
When I fell off the wagon, I got back up to 212-ish, and since Jan 2017 have dropped down to 193.I have been pretty consistently Keto, with just a few “mistakes.”

When did you stop taking Metformin?

@DaveKeto and @richard my feeling is that it is a damn shame Jamie didn’t get tests done just before she started keto again in January as there could well be improvement. I think the best way to see an accurate idea of trend will be to retest in 3-6 months.

I ran out of the metformin… probably in Feb… Since I was Keto, I figured (erroneously… I obviously have too much faith in my body) that it would be ok since I wasn’t consuming sugars/carbs/etc.

I haven’t been to the endo in so long, they won’t refill my meds until I see him. It’s very hard to get into the endo quickly, so I still have another 2 weeks before I’ll even be able to get meds, again.

I would be willing to bet that had a significant impact on BG at least.

Hi Jamie-

This profile definitely doesn’t make sense for standard keto – particularly the high fasting glucose and A1C of 10.5. My first instinct is that you have a “leak” you’re unaware of that is feeding you a lot more carbs than you thought.

Do you track your food carefully?

Alas, I’m very limited in judging medication effects on lipid profiles as well. But to keep the context, about when did you go off your meds before this last blood test?

I don’t track anymore. I have found that I become obsessive when I track food. I feel like I have to “meet” everything and then my focus is food centered and I’ll overeat. When I do more with following actual hunger cures, I don’t tend to eat as much.

Yesterday I had a breakfast pizza w/ bacon, eggs, and a little onion on carl’s dough, topped w/ a little salsa. For dinner I had a small chili from wendy’s and made a chicken ceasar salad… I didn’t much like the salad, though, and probably only ate, at most, a cup of the romaine.

I don’t consume fruit, not even berries. I was telling Daisy, I may have a couple pieces of sugar free candy in a week, and maybe a diet soda or sf ICE drink. I don’t care for the taste of artificial sweetner, so it’s not excessive… Usually just when I’m craving something that I used to have when I ate carbs. Typically, I do find I run a little higher than I have projected I should on protein. When I track, it was usually around 100. Carbs tend to stay around 25 (net). I don’t stress too much if they’re at 30 one day. I did track for a month or so, about a month ago.

Is it possible that my body would still run high w/o carbs, since I’m not taking my meds?

My prior draw was with meds. Then I stopped keto shortly after and avoided the dr, and only took metformin. Trulicity makes you sick if you eat carbs, so I was always feeling pukey. I stayed on the metformin until about a month ago when I ran out. I did not have a panel done w/o meds and on my SAD. I thought I’d be ok w/o meds since I was keto… but, I have been diabetic for years.

I’ll always advocate tracking diet closely when looking for the cause of a dysregulation. It’s a common engineer and manager statement: if you don’t measure it, you can’t manage it.

At a minimum, I’d want you to get a glucometer and start testing your blood pre and post meal (postprandial) so you can help discover where your blood sugar spikes are coming in and how long they are lasting.

You may need to start targeting gross carbs instead of just net. This has been a topic of much debate in the keto community, but I tend to think it’s a course correction if the signals are there.

But without question, from a systems standpoint, there’s too much energy (glucose + triglycerides) being held in your bloodstream and not getting taken up by your cells (muscle or adipose) by your fasting blood draw and I’d consider it worth addressing right away. If you can at least know how much you’re ingesting on a regular basis, you can help determine if it is a external thing (the diet) or an internal thing (metabolizing the energy) and go from there.

But, I will say, last week my mother n law passed away unexpectedly. We were doing a lot of traveling and I did have carbs throughout last week. Some emotional eating, some was really out of convenience. However, that is not a typical behavior for me.
