Should be worried about these results? i have been doing keto for 3 months and lost 30 lbs … feel great but these labs look like this

Retest again at six months, because things might have settled down by then. Your ratio of trigs to HDL is not seriously out of whack, although it would be better if it were under 2.0.

Be careful how your read that paragraph about familial hypercholesterolemia. First, it may not apply to you. Second, half of the people who have the condition never develop heart disease and live perfectly normal lives. The ones who have heart attacks are the people who also have mutations that affect their clotting factors, making the blood likelier to clot. In any case, it is not the cholesterol that is the risk.
