Ketones over 7?

Who has experienced ketones over 7 when starting keto? Literally asking for a friend. My ketones are 1 to 2 usually.

I would worry that there may be inadequate endogenous insulin circulating to control the ketone bodies.
Is your friend a long term T2D?
Does s/he know their insulin level?
Seems high to me but I am not a doc.

Isn’t the ketoacidosis level that worries the doctors above 12? 7 sounds very high to me, I think I would be trying to get it down.

I think Ketoacidosis is more of a concern for T1 diabetecs rather than T2 and usually occurs when ketones are high AND blood sugar levels are high ( I have a feeling Dr Fung said something about this - correct me if I am wrong).

If a T2 D has sustained enough damage to the beta cells in the pancreas (usually happens after many years of uncontrolled T2D) then insulin production can drop right off to low or nonexistent levels.
As insulin is necessary to moderate ketones in nutritional Keto sis this can lead to Keto-acidosis.

Yes can see that this would be a problem for a T2 so far down the line! Any idea how many years this could take?

It is dependant on many variables- how rapidly beta cells are dying, average HbA1c, lots of things.
In my experience as a nurse and then paramedic I generally saw people in this state after decades with poorly managed T2D.

Blood sugar is normal

Mine went as high as 5.6. I was worried about that but they eventually came down.
