Getting what I think to be kidney pain. I got up to walk around and felt a sharp pain on my left back side. My uric acid was 6.5 so I took an extra 200 mg allopurinol. Been taking ibuprofen past couple days as well 600mg at a time (which is known to be hard on kidneys). I also take 2000mg Metformin each day. Between the uric acid build up from recently getting into strong ketosis (1.4 mmol/L BHB is the highest I’ve had in days), along with how hard ibuprofen is on kidneys and the potential lactic acidosis build up of metformin due to impaired renal function… I’m thinking this might be causing the pain. I’ll talk to doctor about taking more than 300mg of allopurinol per day since I am morbidly obese maybe I can, if I brak up the dose throughout the day. I might have to stop Metformin and Ibuprofen both I am thinking. This is a bit scary. Pain is pretty bad right now… about 1/3rd of what it feels like when I had gall bladder attacks before taking out gall bladder. Going to pee through a strainer and into a cup to see if there are any stones or urine cloudy. Any suggestions. (Sorry for the grammar or if I am not very clear… a bit impatient typing as I am in quite a bit of pain right now.)
HELP please -- Kidney Pain, Ketosis, Uric Acid, Ibuprofen, Metformin
Wish I knew how to find a Nephrologist in the Tulsa, OK area, who is familiar with diabetes, metformin side effects, ibuprofen and how it affects kidneys, ketogenic diet, uric acid as well as gout.
From my understanding uric acid competes with ketones for renal tubular excretion in the kidneys, which causes a back up of uric acid. (I have gout and take allopurinol to reduce uric acid.) I took ibuprofen recently as well and I know that is hard on the kidneys. And if there is a kidney impairment it causes a build up of metformin (and I’m taking a lot at 2000 mg)… and if I recall correctly lactic acid then tries to compete with the uric acid and ketones etc… and the ibuprofen…
I would probably seek medical attention to figure what’s causing your kidney pain. From what I understand, high uric acid can be caused by dehydration. Simply drinking more water might bring those levels down. When we restrict carbs, we require more water than we are used to drinking. Two liters per day is a good starting point. Maybe less, maybe more depending on thirst and how often you are having to relieve yourself.
And a word of warning about excessive use of ibuprofen… it can cause gastro-intestinal problems when taken excessively. I know this from personal experience and there’s plenty of info on the web to also confirm it.

I’ll try drinking more water, but I just drank like a large 32 oz jar of it in the morning. I don’t think I’m dehydrated. I eat plenty of salt as well.
Yeah I know about Dr. Fung read all three of his books: The Obesity Code, The Complete Guide to Fasting and Diabetes Code.
I wonder if I subscribed to Dr. Jason Fung’s Intensive Dietary Management program, and did a Forum post there, if I would potentially get some good ideas of what the problem could be?
He seems like the kind of man who would answer your question if he saw it. I truly believe he enjoys helping people.
