Do these test results confirm Insulin Resistance?

I got my blood work back the other day, and I think it shows that I am still Insulin Resistant. I was never tested for Diabetes or IR before starting Keto, but I highly suspect that I was pre-diabetic.

So after following the Keto WOE for nearly 8 months when the blood work was drawn my results are:
Glucose (random, but as I eat OMAD it was fasted), 4.4
HBA1C, 5.8
TSH, 5.79

Urine work
Specific Gravity, 1.004
Glucose, negative
Ketones, 1+

My doctor put me on medication for my Thyroid, and ever since I have felt moody/ irritable (it’s only been 5 days). But this could also just be me…I’m not sure if that is a side effect. He said it would be two weeks before I could feel any changes from the meds.

I am looking for some more guidance for getting on the right track. I know it’s tricky, not having a starting point to compare results to. But I am down about 90lbs, and generally feeling more energetic than before. :wink:

Do you have a result for fasting insulin? An elevated value for fasting insulin would indicate insulin resistance.

What are your HDL and triglycerides?

My random glucose was fasted about 21 hours, I think.

The only way to really know is to get a drink of glucose and take insulin and blood sugar before the drink and then every hour over at least 2 hours or preferably a lot more.

Low HDL and high trigs are markers for IR, but not conclusive. I personally think fasting insulin is next to useless, as I’ve had very low to very high fasting insulin values while low carb the entire time. As a marker, though, I guess it’s possible to say something, and people use it and fasting glucose as a marker of IR. In my case, it’s not a great marker.

HbA1c is OK, but that only tells you what you ate over 60-90 days or so, not how insulin resistant you are.

Fasting glucose and fasting insulin are two different tests. They probably didn’t run a fasting insulin if it wasn’t specifically requested.

One calculation of insulin resistance is the HOMA but you need both fasting glucose and fasting insulin for this. I paid for my fasting insulin myself since my doctor never orders it with regular bloodwork.

Your A1C indicates pre-diabetes I believe.
