Fat Adaption and Ketosis are different.
You could have been in ketosis the whole time and are only just nearing fat adaption, consider adaption like a dimmer up and down rather than an on or off.

No. Mitochondrial adaptation can take a long time to reverse on a heavy carb diet. At the very worst you will gain some water weight temporarily.
