Thank you! I’m glad you enjoyed the episode, I’ll actually be popping up on another podcast episode (Ketogeek) sometime in the following weeks if you wanted to hear me geek out some more
I go much more into depth, especially about small LDL and its possible functional use in the body.
As far as fasting goes I was more asking if you had eaten before the test (nonfasted) or fasted for multiple days because both can skew results in different ways. I see in another reply you had fasted 12 hours which is what I wanted to make sure of.
So you have been sick… yes I think that would certainly influence your results and make them look “off” for a while. I got a test when I was in the middle of being sick and my inflammation markers were off the charts (CRP of 48! ferritin of 250+! small LDL of 1800+! triglycerides over 110!) so I did exactly the same as I had been (carnivore, dairy) and checked back three weeks later.
Result of doing nothing: CRP of 0.83, ferritin of 92, small LDL had dropped by over 400 to 1400, TG had dropped a bit. Check back in another week: CRP at 0.56, ferritin at 70~, small LDL at 800~, triglycerides at <100.
So it appears as though getting a blood test while sick had majorly messed up my results and a month later they are still returning to baseline (I’ll be getting another test in a month to confirm).
This has been noted in studies too - that getting sick can majorly mess up your results and give you “bad” markers, but I would argue that this is a normal response and as long as it abates as you get better it is a proper immune response at work.
Your results of >3500 LDL-P, high small LDL, etc all look exactly as mine did when I was sick. I would really wonder if it wouldnt start to show a downward trend in say a month like mine did.
As far as small LDL goes, it would probably count as pattern B, but this does happen with infection/illness and if that is the cause it is temporary and may not be of concern if transient.
Where I get concerned is when you have sources of inflammation, damage, etc that are chronic and keep going and going over months and years not a temporary normal thing like getting the flu. My view, after looking at the pathways, is this is a system for dealing with clean up. Smaller cleanups every now and then are easily handled, but bigger messes that keep going for years never get properly dealt with and the mechanisms built to handle it eventually start to reach a point of getting overwhelmed and then you get things like atherosclerosis etc…
That is the impression I’ve gotten so far anyway, but I’m no expert.
Anyway, sounds like you have a plan! If you get a follow up test be sure to @ me. I hope you see improvement, and find what works for you! That is always the most important thing
